Abstract
Objective: This study uses various hematological inflammatory parameters to evaluate the diagnostic distinction between testicular torsion (TT) and epididymo-orchitis (EO). These include the neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, lymphocyte-to-monocyte ratio, systemic immune-inflammation index, systemic inflammation response index, panimmune inflammation value, derived neutrophil-to-lymphocyte ratio, and neutrophil-lymphocyte-platelet ratio. Methods: The demographic data and hemogram parameters of TT and EO cases treated in the pediatric surgery clinic between January 2010 and January 2023 were retrospectively examined. The hematological inflammatory parameters were statistically compared across the groups. Results: The study included 181 patients, categorized into control (n = 60), TT (n = 64), and EO (n = 57) groups. The mean ages were 9.80 ± 2.07, 13.38 ± 4.26, and 10.95 ± 4.07 years, respectively. The neutrophil-to-lymphocyte ratio (p<0.001), the lymphocyte-to-monocyte ratio (p=0.028), the systemic inflammation response index (p<0.001), the systemic inflammatory index (p=0.002), the panimmune inflammation value (p=0.006), derived neutrophil-to-lymphocyte ratio (p<0,001), and neutrophil-lymphocyte-platelet ratio (p<0,001) were found to be statistically significant in distinguishing TT from EO. While Platelet-to-lymphocyte ratio (PLR) (p = 0.144) was not statistically significant. Conclusion: Hematological inflammatory markers obtained from a routine complete blood count can serve as useful and cost-effective parameters in the differential diagnosis of acute scrotal conditions such as TT and EO.
